  That is a High Standard M200, look for choke tubes at the gun auction sites or E Bay. You probably want a full choke for trap and imp. cyl. for skeet.

  It was made by High Standard. Look for a poly choke and you'll find what you need. Probably.

  The Power Pac choke system was made by Pachmayr. Check ebay or gun auction sights for the part you want.

  Mod choke for a 20 on skeet may just be the right choke to use.
